Thank you for your interest in helping Lawson Software ("Lawson") to: (i) maintain world-class operations, ethics and values by reporting violations or possible violations of the company's Global Business Code of Conduct ("Code of Conduct"), Employee Reference Guide ("ERG"), or questionable accounting or auditing matters, disclosures of Protected Information and (ii) comply with the privacy and security regulations promulgated under the Health Insurance Portability Act ("HIPAA") and Gramm-Leach-Bliley ("GLB").
The reports below should be used to report any disclosure or viewing of Protected Information. Again, thank you for your assistance in helping Lawson and its clients to comply with the requirements of HIPAA.
This site has been designed to report any disclosure or viewing of Protected Information. This email address does not accept reports of other behavior that may constitute a violation of other Lawson policies. Any reports of violations of other Lawson policies must be made in accordance with the policies outlined in Lawson's ERG.
Protected Information Tracking Report: This report is for contractors and employees to keep record of what protected health and financial information they have seen while working. Employees and contractors who fill out this form were supposed to see the protected information they have seen, as it was necessary to do their job.
Protected Information Violations Report: The purpose of this form is to report any unauthorized viewing of protected health and/or financial information. This can occur accidentally or illegally and be done by employees, contractors, or third parties.
